Losing my milk

Is it possible for a mother who has worked feverishly for 5 weeks to increase her milk supply to ultimately lose her milk?

My lactation consultant that worked with me in the hospital told me (after weighing and re-weighing my daughter) that I was losing my milk. My daughter was losing weight and after re-weighing her after she nursed, informed me that she was only getting 2cc's of milk. She recommended that I rent a hospital grade breast pump to help extract all my milk and increase my supply.

I bought and read a book entitled "A mother's guide to Increasing Milk Supply" and subsequently tried power pumping (pumping every hour and pumping immediately after nursing even through the night), I've been taking Fenugreek tablets, Malunggay tea tablets, I drink Mothers Milk tea, I drink nothing but water, and I eat steel cut oats at least twice a week (which I'm told helps).

I have never pumped more than 1 and 1/2 ounces (1 ounce from my right breast and 1/2 from my left) in a sitting. Usually its a little less than that. Even after my daughter nurses on me 5-6 minutes per breast, I still pump about an ounce collectively from my breasts.

I was told that pumping more often would convince my body that I needed more milk, maybe even cause me to produce enough milk to feed twins! My daughter eats 2+ ounces per feeding and if I'm pumping an ounce even after she nurses on me, she's not getting nearly enough from me, so she always gets a bottle afterward (either breastmilk or formula.

My lactation consultant says my daughter latches very well and has a normal suck (her cheek muscles flex and relax well, her lips are flanged as they should be, etc.) even though I supplement her with a bottle.

I've been pumping as I always have the past couple days and the amount I produce is becoming less and less, seemingly out of nowhere. My daughter is becoming frustrated on my breast and whips her head side to side and cries. I get her latched back on and she'll start sucking again, but after a few minutes she'll start crying again as if she's not getting anything.

I gave her a bottle of formula and pumped for 10 minutes only to discover that I only produced just over half an ounce out of both breasts! That's more than half of what I was producing this time last week! I was frustrated, so I pumped every 1-2 hours to stimulate my breasts and never got more than half an ounce each time...

My daughter has never had nipple confusion, she still has a good latch and a strong suck, I'm not eating anything I shouldn't be, I don't drink or smoke, I'm not under any new stresses...

What's wrong with me??

This is a VERY difficult question to answer in this format. I am not able to ask you medical questions and anatomy questions about you and baby. I can think of dozens of possible scenerios.
