Plugged Ducts or Mastitis

Hello,
I have a question regarding Mastitis and (or) plugged ducts.
When I was pregnant with my first child ( he is 2 now), I think I've experienced Mastitis or plugged milk ducts , and the reason I say " I think" is because I've diagnosed myself online ( my physician hasn't really helped me much with my problem, he didn't know what that was)
I've breastfed my child for the first 5.5 months. I experienced mastitis or Plugged Ducts everyday during the five months of feeding ( inflammation, swelling, redness, tenderness and pain with low milk flow) I breastfed him every 3 hours, I even got the Madela electric pump, and I still needed to massage and shower with hot water twice - 3 times a day to soften my breasts.
The low flow of milk had caused my baby to lose weight so I gave him formula with my breastmilk. I know I had a low flow of milk because my son was always hungry even if I feed him for over 30 mins, and with the Madela pump I pumped 1oz in 45 mins.
Ok my questions is: as you can tell I am not sure what was the problem exactly but I am pregnant with twins and I am worry that problem would happen to me again . So I was wondering since this happened with my first child does it mean it will happen again with my second? ( i don't want to take any risks since I'll have 2 babies) so should I go ahead and have formula ready when they are born? or is there is a way I can fix this?
I really would rather to breastfeed but I am lost and have no one to help me here. I am located in Cairo Egypt. They don't have lactation consultants here for some strange reason and my Gyno didn't know what I was talking about when I told him my issue. All what he cared about if I had fever and I didn't.
