Putting baby back to the breast after pumping breast milk for 3 months

I gave up breastfeeding when my baby was 2
Months old due to issues I was having. Baby was gaining weight but would be fussy at breast would feed for over an hour every time and seem hungry with in no time. I had issues with red swollen nipples and position issues. I started pumping and as it only took 20 minutes to pump and covered her feeds most of the time I was happy. 3 months on my supply is only covering 65-70% of her feeds and this is with pumping 7-8 times a day. I also use many tricks which are meant to help my supply like eating oats and taking supplements like mothers milk. My baby also sleeps through the night but I get up to pump 2-3 times.
I would however like to see if I can get the baby back to the breast. She will be starting to wean in 5 weeks so would like to drop the formula I'm having to give and hopefully be able to breastfeed then Exclusively.

Note from the lactation consultant
It is very possible to get the baby back on the breast. The best way to do this is to work directly with a lactation consultant or la leche league leader IN PERSON. The babies latch needs to be assessed then the support person can help you with getting the baby back on. It may require the use of tools such as a supplemental feeder or a nipple shield.
Support from a breastfeeding expert is imperative to success.
